Use your new MetLife card starting 7/1/24

Then starting 7/1/24, switch to using your new MetLife card. See below for the new plan benefits and updates.

This transition is occurring as Nationwide has decided to no longer underwrite the dental coverage available as a benefit of your membership in the National Small Business Association. We have worked very hard to ensure a near seamless transition for you to ensure your continuity of care.

To make this happen, we're partnering with MetLife, a leading global insurance provider known for its high-quality dental programs, to insure your dental coverage under a policy issued to the National Wellness and Fitness Association (NWFA). This collaboration brings together MetLife's expertise in dental insurance with NWFA's commitment to enhancing health and quality of life for its members.

This transition ensures the continuation of the excellent dental coverage you're accustomed to designed to support your health journey and save you money. Rest assured; you'll continue to receive the exceptional member services you’ve always had from all of us at NCD.

Your existing dental coverage, currently underwritten by Nationwide through a policy issued to the National Small Business Association (NSBA), will conclude on June 30, 2024. Without the arrangements we’ve made, the discontinuation could have left you trying to find new coverage from an individual plan, potentially at higher costs and with fewer benefits. To ensure continuity and enhance your dental insurance benefits, MetLife, in collaboration with NWFA and facilitated by NCD, will take over your coverage. We're enthusiastic about this new arrangement and the comprehensive benefits it brings, ensuring a smooth transition for you without any coverage lapse.

As part of this transition, your membership grants you exclusive access to exciting discounts and benefits on dozens of health, wellness, personal, and travel services through the NWFA.

We have good news
about your dental coverage!

Will my dental coverage benefits change?
Your dental coverage and benefits are staying the same. For detailed information about your coverage, please refer to welcome kit you have received in the mail.

Will my waiting period for major services start over?
No. You will receive full credit for any time accumulated toward major services. The benefit accumulation period you’ve already built up with your Nationwide plan will continue seamlessly, and there will be no reset.

When should I start using my new MetLife ID card?
Continue using your current ID card for the Nationwide plan through June 30, 2024. Starting July 1, 2024, please begin using your new MetLife ID card for all dental services.

Which network should I refer to when visiting my dentist?
Until June 30, 2024, you should refer to the Careington Maximum Care Plus PPO network through your Nationwide dental plan. Beginning on July 1, 2024 and moving forward, please refer to the MetLife PDP Plus network.

How do I find an in-network dentist under the new MetLife plan?
To find a MetLife in-network provider, visit NCD.com/MetLifeDentists or contact our Member Care team for assistance by calling 800-202-8380 or emailing MetLife.MemberCare@NCD.com.

What if my current dentist is not in the MetLife network?
We recommend using the MetLife network to minimize out-of-pocket costs. You can search for providers at NCD.com/MetLifeDentists or contact our Member Care team for assistance at 800-202-8380. Just like it always has been with your NCD dental underwritten by Nationwide plan, if you use an out-of-network dentist, we will pay the same amount as if you were in-network. Since we would not hold a contract with the dentist you used, they are free to charge you over and above the amount your NCD by MetLife plan covers. This means you are likely to pay more if you use an out-of-network dentist.

How do I ensure my dentist knows about the change in my insurance?
Inform your dentist of the change in your insurance plan during your next visit. Provide them with your new MetLife ID card which has the new claims address starting July 1, 2024.

What claims address should I use for dental services?
For any dental services before July 1, 2024, use the Nationwide plan claims address:

Merchants Benefit Administration (MBA)
PO Box 981640
El Paso, TX 79998.

For services on or after July 1, 2024, use the MetLife claims address:

Group Dental Claims
PO Box 981282
El Paso, TX 79998.

Who do I contact for questions during this transition?
For any questions or assistance, please contact our Member Care team at 800-202-8380 or MetLife.MemberCare@NCD.com. This team is set up to help with all inquiries related to the transition to your new MetLife plan.

How do I modify my plan?
Should you have any concerns or wish to modify your plan, you can do so by contacting member care at 800-202-8380 or emailing us at
MetLife.MemberCare@NCD.com.
